Change of Name Deed Information

A Change of Name Deed (Deed Poll) is a legal document that records a person's intention to abandon their old name and use a new name for all purposes.

For adults (18 years and over):

• You must attend the appointment in person.
• Bring valid proof of identity and proof of your current address.
• The new name must not be intended for fraudulent or misleading purposes.

For children under 18:

• A person with parental responsibility must complete the application.
• All individuals with parental responsibility should provide their consent to the change of name.
• If consent from another person with parental responsibility has not been obtained, the appointment may still proceed; however, the change of name may be disputed and could be considered by a court if there is a disagreement.
• We cannot provide legal advice. If you are unsure whether you have the authority to proceed, you should seek independent legal advice.

Please bring:

• Proof of identity for the applicant.
• Proof of address.
• Child's birth certificate (for child applications).
• Any relevant court orders or documents relating to parental responsibility, where applicable.

Important: A Change of Name Deed does not automatically update your records with government departments, banks, schools, or other organisations. You will be responsible for notifying these organisations and providing copies of your completed deed where required.
